{"id":15352,"date":"2024-05-31T10:29:39","date_gmt":"2024-05-31T14:29:39","guid":{"rendered":"https:\/\/www.woodsworthcoop.ca\/?p=15352"},"modified":"2026-06-07T09:49:57","modified_gmt":"2026-06-07T13:49:57","slug":"proof-of-principal-residence-for-internal-membership","status":"publish","type":"post","link":"https:\/\/www.woodsworthcoop.ca\/index.php\/proof-of-principal-residence-for-internal-membership\/","title":{"rendered":"Proof of principal residence and ID requirements for membership"},"content":{"rendered":"<p data-start=\"162\" data-end=\"220\"><strong data-start=\"166\" data-end=\"220\">Membership Documentation Requirements \u00a0for Woodsworth membership<\/strong><\/p>\n<p>from: Membership Bylaw (Member Approval and Unit Allocation Bylaw), Attachment B.<\/p>\n<p data-start=\"222\" data-end=\"392\">To receive a membership application form at a membership information session, <strong data-start=\"269\" data-end=\"289\">long-term guests<\/strong> and <strong data-start=\"294\" data-end=\"309\">Turning 16s<\/strong> (aged 16 to 19) must show proof that <strong data-start=\"348\" data-end=\"391\">Woodsworth is their principal residence<\/strong>.<\/p>\n<p data-start=\"222\" data-end=\"392\">These applicants must bring:<\/p>\n<ol>\n<li style=\"list-style-type: none;\">\n<ol>\n<li>Applicants must bring photo ID.<\/li>\n<li>Long-term guests must also bring a copy of their signed and witnessed (by staff) long-term guest agreement.<\/li>\n<li>Two (2) types of documentation from the following list must be presented to prove Woodsworth is their residence.<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n<p data-start=\"556\" data-end=\"582\"><strong data-start=\"560\" data-end=\"582\">Accepted Documents<\/strong><\/p>\n<p data-start=\"583\" data-end=\"697\">Documents may be <strong data-start=\"600\" data-end=\"635\">paper or digital (screen print)<\/strong>, unless otherwise stated.<br data-start=\"661\" data-end=\"664\" \/><strong data-start=\"664\" data-end=\"696\">Photocopies are not accepted<\/strong>.<\/p>\n<p data-start=\"699\" data-end=\"710\"><strong data-start=\"699\" data-end=\"707\">Note<\/strong>:<\/p>\n<ul data-start=\"711\" data-end=\"860\">\n<li data-start=\"711\" data-end=\"790\">\n<p data-start=\"713\" data-end=\"790\">The Membership Committee will not retain any copies of these documents.<\/p>\n<\/li>\n<li data-start=\"791\" data-end=\"860\">\n<p data-start=\"793\" data-end=\"860\">The Woodsworth Board may request to see the original documents.<\/p>\n<hr \/>\n<\/li>\n<\/ul>\n<h5>This list is part of the Membership \u00a0Bylaw, Attachment B.<br \/>\nThe Board enhanced the list in June 2024.<\/h5>\n<h5><b>List to the information session for membership &#8211; two (2) items required:<\/b><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Valid Ontario driver\u2019s licence (or valid license with previous address and a temporary license with Woodsworth address). An application for a change of address is not accepted.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Valid Ontario Photo Card (only if a driver\u2019s licence is not presented)<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Ontario vehicle permit (ownership) (green form) with Woodsworth address<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Bank statement with name and membership applicant\u2019s address. (recent)<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">A major credit card (show physical card) plus a copy of a recent statement for that credit card. (Counts as 1 item.)<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Income tax Notice of Assessment or T4 (recent)<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Statement of direct deposit for Ontario Works (recent)<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Ontario Disability Support Program (ODSP) (recent)<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Statement of Employment Insurance Benefits Paid (T4E) (recent)<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Statement of Old Age Security (T4A) (recent)<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Statement of Canada Pension Plan Benefits (T4A) (P) (recent)<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Workplace Safety and Insurance Board Statement of Benefits (T5007) (recent)<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Canada Pension Plan Statement of Contributions<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Ontario Student Assistance Program (OSAP) correspondence<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Recent correspondence from the Federal or Ontario governments addressed to the membership applicant at Woodsworth, received within in the last 3 months. (Not forwarded mail)<\/li>\n<li aria-level=\"1\">Any of the following recent statements from a bank, trust company or credit union: TFSA RRSP, RRIF, RHOSP statement<\/li>\n<\/ul>\n<p><strong>Turning 16s: 16-19 year olds may present any of the above for long-term guests or any of the following:<br \/>\n<\/strong><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\">School, college, university report card or transcript or letter from the school or institution on letterhead showing applicant&#8217;s name and Woodsworth address<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Employer record (e.g. pay stub, letter from employer on company letterhead)<\/li>\n<\/ul>\n<h5><strong>PHOTO ID:<\/strong><\/h5>\n<p data-start=\"78\" data-end=\"267\">Additionally, membership candidates attending an information session must show <strong data-start=\"157\" data-end=\"217\">one piece of government-issued or institutional photo ID<\/strong> with their name.
